The following links provide authorized access to the add/edit forms and various reports generated by the various database tables that are components of this project.

1. Reports

1.1 Parish basic report: Provides a searchable database listing / report of selected core information about each UOC parish.

1.2 Parish status report; Provides information on the status of each parish regarding what data has been found, and what data still needs to be entered into the various database tables.

1.3 Clergy base report: Provides a searchable database listing / report about demographic and historical information on each priest.

1.4 Clergy education / training report: Provides listings and reports on the education attainment and theological trainings of each priest.

1.5 Clergy assignment report: Generates information about where and when each priest served.

1.6 Cemetery report: A report with information on cemeteries associated with UOCC parishes.

2. Add/Edit Forms

2.1 Parish form: Used to input, edit and manage the core data elements associated with Ukrainian Orthodox parishes.

2.2 Clergy base form: Used to input, edit and manage basic demographic data about each priest.

2.3 Clergy education form: Used to input, edit and manage educational and theological training taken by each priest.

2.4 Clergy assignment form: Used to input, edit and manage data regarding where and when each priest provided religious services.

2.5 Cemetery form: Used to imput, edit and manage data about UOCC parish cemeteries.
